Home Health Field Nurse (RN)

We are seeking Home Health RN Case Managers for our five-star-rated skilled Home Health program, serving Allegheny, Beaver, Butler, Westmoreland, and Washington counties. Both full-time and PRN positions are available. The RN Case Manager works under the general direction of the Medical Director and/or attending physician to provide skilled nursing assessment, planning, and coordination of services to maximize patient and family well-being. Qualifications: • Graduate of a nursing school accredited by the National League of Nursing; baccalaureate degree preferred • Registered Nurse licensed in the State of Pennsylvania • Pennsylvania driver's license, current auto insurance, and access to a car during working hours • Home Health experience preferred • Excellent assessment skills and ability to work independently • Strong communication skills and ability to work effectively in a team • Ability to lift and push/pull a minimum of 50 pounds Benefits: • Up to five weeks of paid time off in your first year • 401(k) with matching • Health, dental, and vision insurance • Life insurance and Employee Assistance Program • Referral program EOE.
