Intake Case Manager, CI

Gesher Human Services is a bridge to hope and opportunity for people at work, at home, and in the community. Gesher's workforce development, behavioral health, and inclusion programming serves all Metro Detroiters while meeting the needs of the Jewish community. The Intake Case Manager is responsible for recruiting individuals with disabilities for Community Inclusion services (skill building/supported employment) in Wayne and Oakland Counties. This role includes outreach to schools, funders, and referral sources, as well as conducting intake meetings and ensuring all documentation requirements are met. Key Responsibilities: • Cultivate referral relationships with schools, funders, and community partners; plan targeted recruitment events • Respond to referral inquiries promptly and oversee tours and intake meetings • Complete intake packets, develop plans, and provide interim case management support • Train staff on each new participant's Individual Plan of Service (IPOS) • Collaborate with program leadership to ensure appropriate service placement and financial resources • Using a person-centered approach, develop and implement services that promote a healthy, active, and socially integrated lifestyle • Monitor and periodically review support plans; secure and maintain required service authorizations • Prepare timely documentation in computerized databases for funding source compliance • Transport participants using agency or own vehicle as needed Qualifications: • Bachelor's degree in psychology, social work, special education, or related field required • Two years of related work experience serving individuals with disabilities required • Reliable transportation and valid Michigan driver's license • Strong interpersonal and communication skills • American Sign Language fluency is a plus • Complete all required trainings including CPR, First Aid, and Recipient Rights Gesher is an equal employment opportunity and affirmative action employer committed to diversity and inclusion.
