Elan Skilled Nursing and Rehab is seeking a Social Worker to join our dedicated team. This is a full-time, hourly position. Benefits: • 403(b) plan • Medical, Dental, and Vision Insurance • Holiday Pay, Sick Time, Vacation Time, and Personal Time Responsibilities: • Develop ongoing relationships with residents and their families • Interview and assist residents and family members with personal, family, financial, and physical or mental health challenges • Secure information such as medical, psychological, and social factors contributing to residents' situations • Counsel residents individually, with family, or in small groups; aid residents in mobilizing inner capacities and community resources • Participate in Interdisciplinary Care Plan meetings and ensure psychosocial goals are incorporated • Maintain compliance with regulatory agencies for assessments and documentation • Provide information, tours, and assistance to prospective residents and families • Establish contact with local hospitals; visit hospitalized residents as appropriate • Refer residents and families to community resources and organizations • Assist with and coordinate admissions as assigned Qualifications: • Current license under the Social Workers, Marriage and Family Therapists and Professional Counselors Act; or Bachelor's or Master's degree in Social Work from an accredited institution • Ability to read, analyze, and interpret professional journals, governmental regulations, and procedure manuals • Strong interpersonal skills and ability to engage with diverse populations Our organization is guided by Jewish values of respect, compassion, advocacy, quality, and innovation in all areas of care.
