Job description
Jewish Family Service LA (JFSLA) is a comprehensive network of services that welcomes individuals and families of all ages and identities. The Social Worker II is responsible for conducting in-depth intakes and assessments covering psychological, social, physical, and environmental concerns for frail, elderly clients residing in the City of Los Angeles, Holocaust survivors and their families. This role evaluates clients, identifies limitations which impede independent living, and activates resources to allow clients to remain safely in their home setting. Qualifications: • Master's Degree in social work or a related field • Registered with the California Board of Behavioral Sciences or Board of Psychology; licensed preferred • Current unrestricted CA Driver's License and proof of auto insurance • Experience and interest in providing care to older persons required • Care management and community-based service delivery experience required • Knowledge of community resources and the aging process • Bilingual in Farsi, Russian, or Spanish is a plus • In-home visit experience strongly preferred • Computer literate in MS Office (Word, Excel, Outlook) Key Responsibilities: • Conduct in-depth intakes and assessments for City of LA clients and Holocaust survivors • Develop comprehensive care plans and coordinate delivery of all services • Respond to intake callers and follow up on client situations • Offer crisis intervention and short-term or ongoing psychotherapeutic support as needed • Identify and develop family and informal support systems • Prepare reports for DOA III-B funding requirements • Maintain timely documentation in Evolv database JFSLA is committed to cultivating a culture of inclusion and connectedness.
