Team Leader/Social Work Supervisor - Community Guardianship Program

Responsible for leadership of an assigned team, ensuring that all client needs are being met and that all New York State Supreme Court orders, contractual obligations, and agency policies are being met. Principal Responsibilities: • Consult with and keep Program Managers informed of activities, needs, and problems related to case management, staff issues, program mandates, and court orders. • Supervise all members of team including caseworkers, accountants on client fiscal issues, and team coordinators. • Evaluate performance of staff periodically with at least one written review per year. • Initiate new client case conferences and assist team with development and implementation of service plans. • Collaborate with physicians, psychologists, nursing home and other health care personnel in client evaluation and treatment. • Coordinate emergency coverage rotation and serve as supervisory backup for on-call social workers (24hrs/day, 7days/week). • Assign new cases in consultation with Program Director and Assistant Program Director. • Monitor Medicaid and Medicaid home care certifications and recertifications of assigned clients. • Ensure all clients are seen at least once per month per contractual obligations. Salary Range: $67,500 - $69,900 per year commensurate with experience. Minimum Qualifications: • MSW or equivalent; or four to ten years related experience, or equivalent combination of education and experience. • Supervisory experience preferred. • Computer literate; ability to work with a range of programs including financial database software, spreadsheets, and Microsoft Suite. • Excellent verbal and written skills. • Able to multi-task and work independently with great attention to detail.
