Role: Case Manager – Washington, D.C. & Greater Washington Area Benefits: flexible work schedule, 401(k) with employer matching, health insurance, investment in professional development WHY THIS ROLE MATTERS: Families facing pediatric illness are navigating one of the most difficult chapters of their lives. The Case Manager is the person who stands beside them through it all - helping them think clearly in moments of crisis, connecting them to resources they didn't know existed, and ensuring they never feel alone in the process. This role is deeply relational. This is not a traditional desk job. One day might involve sitting with a parent who needs guidance navigating a new diagnosis; another might involve coordinating services, meeting with community partners, or helping a family access a critical resource. The Case Manager will serve as the primary support professional for approximately 50 families, ensuring each family receives thoughtful, consistent, and personalized support. For the right person, this is an opportunity to do meaningful work while helping build a stronger support network for families across the Washington, D.C. region. Responsibilities Include: • Serve as a steady, trusted guide for families navigating complex pediatric diagnoses - building deep relationships that ensure no parent or child feels alone in moments of uncertainty or crisis • Coordinate and deliver personalized support plans that integrate medical, emotional, and practical needs - bridging gaps between hospitals, schools, and community resources • Build and maintain a robust network of providers, specialists, and community partners - creating a high-quality referral ecosystem that connects families to the right support at the right time • Advocate on behalf of families in healthcare and educational settings, helping them access critical services, ask the right questions, and make informed decisions • Respond to urgent, real-time needs with compassion and precision - mobilizing resources quickly and effectively when timing matters most, while empowering families toward long-term stability and independence Required Experience: A healthcare-related master's degree (such as social work, human services, public health, or healthcare navigation) is preferred but not required.
