Hospice - Admissions Nurse

JSSA's hospice program, one of the top hospice programs in Montgomery County, has an immediate opening for a full-time Admissions Nurse. The program provides patients and their families with end-of-life care that supports dignity and comfort, gives families peace of mind, and honors cultural traditions and customs.

Responsibilities:
• Perform initial assessments of patients' needs and obtain fully informed consents before admitting patients to the program
• Serve as a clinical resource for referral sources, patients and family members
• Facilitate end-of-life conversations and decision-making with the family or caregiver
• Prepare and discuss an initial plan of care with the physician
• Deliver hospice and palliative care services to patients in their place of residence

Qualifications:
• Active RN license to practice in the State of Maryland; CHPN certification preferred
• Minimum of 3 years of acute care and/or hospice experience
• Knowledge and skills to provide meaningful care to and communicate with the terminally ill
• Proficient with Microsoft Office 365 and EMR systems; familiarity with Netsmart/My-Unity or Homecare Homebase a plus
• Valid driver's license and reliable, independent transportation for patient visits throughout Montgomery County

**Hours:** Four 10-hour days per week.

Salary: Starts at $90,000, with medical, dental and vision coverage, a 403(b) retirement savings plan with match, and professional development opportunities.
