Job Summary:
As a Medical Assistant at Carease Health - NJ, you will play a vital role in our comprehensive preventative care program, incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM). You will provide exceptional patient care, taking vital signs, and collaborating with our experienced clinical team to ensure seamless alignment with our patients' needs.

Responsibilities:

• Take vital signs, perform basic medical procedures, and assist with patient care
• Collaborate with our clinical team to develop and implement individualized care plans
• Utilize technology and electronic health records to document patient information and track progress
• Provide education and support to patients, families, and healthcare providers
• Maintain accurate and up-to-date patient records
• Work efficiently in a fast-paced environment, prioritizing tasks and managing multiple responsibilities

Requirements:

• 2+ years of experience as a Medical Assistant or Certified Nursing Assistant (CNA) in a healthcare setting
• Certified Medical Assistant (CMA) or Certified Nursing Assistant (CNA) certification
• Reliable and efficient in taking vital signs
• Strong organizational skills and ability to work in a high-volume setting
• Ability to relocate before starting work
