Job Summary:

Carease Health - NJ is seeking a reliable and experienced Medical Assistant or Certified Nursing Assistant (CNA) to join our team! As a Medical Assistant, you will play a vital role in our preventative healthcare program, focusing on remote patient monitoring, chronic care management, and psychiatric collaborative care. With a strong passion for patient care, excellent organizational skills, and the ability to work efficiently in a fast-paced environment, you will help us improve the lives of nursing home patients and reduce hospitalizations.

Responsibilities:

• Take vital signs accurately and efficiently

• Assist healthcare professionals with patient care and procedures

• Maintain accurate and detailed records of patient information

• Collaborate with the clinical team to develop and implement individualized care plans

• Provide exceptional patient education and support

• Participate in quality improvement initiatives to enhance patient care and outcomes

Requirements:

• Medical Assistant: 2 years of experience in a healthcare setting

• Vital signs: 2 years of experience in taking vital signs

• License/Certification:

  • Driver's License (Required)

  • Certified Medical Assistant (Required)

• Ability to relocate before starting work

Schedule:

• Monday to Friday
