Job description

Director of Patient Services (DPS) – Home Care

Monsey, NY | On-Site (Hybrid Possibility)
$175,000–$225,000

Overview

A large, established home care organization is seeking an experienced Director of Patient Services (DPS) to oversee its clinical operations, lead its nursing teams, and ensure the highest standards of patient care and regulatory compliance.

The organization serves thousands of patients across New York State and operates both Licensed Home Care Services Agency (LHCSA) and Private Duty Nursing (PDN) programs. This position will oversee the clinical functions of both divisions, with responsibility for nursing leadership, clinical processes, compliance, and quality of care.

We’re looking for someone who really knows home care, understands the clinical and regulatory side of the business, and can take ownership of a large clinical operation without needing to be taught the industry.

This is a significant leadership opportunity, but the right person doesn’t necessarily need to have held a DPS title before. What matters most is strong home care knowledge, sound judgment, the ability to lead people, and a proven track record of getting things done.

The organization has an established infrastructure and support team in place. The DPS will have the resources needed to succeed while taking responsibility for clinical oversight, accountability, and continued improvement.

Key Responsibilities

  • Oversee clinical operations across the organization’s LHCSA and Private Duty Nursing programs

  • Lead and manage approximately 10–15 clinical and administrative team members, including field nurses and office staff

  • Ensure compliance with applicable home care regulations, clinical standards, and documentation requirements

  • Oversee clinical audits, regulatory reviews, and survey preparation, ensuring the organization remains audit-ready

  • Develop, review, and strengthen clinical policies, procedures, and workflows

  • Provide guidance and oversight to nursing staff, ensuring consistent quality of care and proper clinical practices

  • Address clinical issues, escalations, and operational challenges as they arise

  • Monitor clinical documentation, field visits, and staff performance to ensure accuracy and accountability

  • Work closely with executive leadership and other departments to coordinate clinical operations

  • Identify opportunities to improve processes, efficiency, and overall clinical performance

  • Support the continued development and expansion of the organization’s Private Duty Nursing services

  • Visit additional office locations as needed to support clinical oversight and audits

Ideal Candidate

  • Active RN license — required

  • Strong, direct experience in home care, preferably within a LHCSA — required

  • Solid understanding of home care regulations, clinical compliance, documentation, and audit requirements

  • Ability to develop policies, manage regulatory challenges, and oversee clinical processes

  • Strong leadership and management skills, with the ability to guide teams and hold people accountable

  • Approachable, collaborative personality with strong interpersonal skills

  • Confident and capable of making decisions and getting results without being aggressive or difficult to work with

  • Highly responsible, organized, and proactive, with strong follow-through

  • Able to recognize problems, develop solutions, and ensure they are implemented

  • Previous DPS or clinical leadership experience preferred, but not required for someone with the right home care background and capabilities

  • Experience with Private Duty Nursing is a plus

  • Comfortable working primarily on-site in Monsey, with occasional travel to other locations

We’re looking for someone who combines strong clinical expertise with the ability to lead people effectively — a knowledgeable, capable professional who takes ownership, works well with others, and makes sure things get done.
