Responsibilities
- Assess patients and provide care within your scope of practice
- Conduct on-site and telehealth visits
- Develop care plans and coordinate follow-up
- Complete and finalize clinical documentation promptly
- Communicate clearly with patients and the care team
Qualifications
- Active PA or NP license
- Currently enrolled and credentialed to bill Medicaid for the services provided
- Strong clinical judgment and patient communication skills
- Comfortable with electronic health records and telehealth visits
- Reliable, organized, and attentive to documentation
