Clinical Case Manager

Alexander Jewish Family Service (Alexander JFS) in Houston is hiring a Clinical Case Manager for its Coaching & Case Management team. You'll provide trauma-informed clinical and case management services to individuals and families facing mental health challenges, combining assessment and therapy with hands-on resource navigation so clients can reach their goals.

Responsibilities:
• Conduct timely assessments, provide brief intervention and stabilization, and refer and follow up
• Complete clinical assessments and DSM-5 diagnoses, and develop individualized treatment plans
• Provide psychotherapy to individuals, couples, families and groups
• Coordinate care with internal programs and outside mental health, medical, employment and social service providers
• Help clients navigate community resources, with home or community visits as needed
• Maintain a weekly caseload of billable sessions and see walk-in clients when possible
• Track grant deliverables, take part in quality improvement, and deliver community trainings
• Keep timely, accurate documentation and handle medical record requests confidentially

Qualifications:
• Master's degree in social work, counseling or a related behavioral health field
• Current LCSW
• Experience diagnosing mental health and substance use disorders using DSM-5
• Experience with individual, family and group psychotherapy and trauma-informed care
• Case management or grant-funded program experience
• Occasional evening, weekend and after-hours availability
