About the Role:

As an Adult Nurse Practitioner with Carease Health - WI, you will play a crucial role in our comprehensive preventative care program, which incorporates Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM). You will work alongside our experienced clinical team to provide proactive care to patients, focusing on both physical and mental health. This role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities:

• Provide high-quality preventative care to residents in skilled nursing facilities

• Conduct on-site visits and remote work from home

• Work collaboratively with our clinical team to provide proactive care to patients

• Build strong relationships with facilities and maintain a patient-centered approach

• Document patient information accurately and efficiently with the support of our scribe team

Requirements:

• Active NP license in the state of Wisconsin

• 1+ year of experience in:

  • Geriatric care

  • Documentation review

• Excellent communication, collaboration, and teamwork skills
