As an Adult Nurse Practitioner (NP) or Physician Assistant (PA), you will deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities:

• Provide patient-centered care to residents in skilled nursing facilities

• Conduct on-site visits and remote work from home

• Utilize RPM, CCM, and CoCM to optimize patient care

• Build strong relationships with facilities and families

• Collaborate with a dedicated scribe team for efficient documentation

Requirements:

• Active NP or PA license in the state of practice

• 2+ years of experience in Geriatric care or Internal Medicine

• Excellent communication, collaboration, and teamwork skills

• Ability to maintain patient confidentiality and adhere to HIPAA guidelines
