Home Care Registered Nurse - Home Care

OB DUTIES / RESPONSIBILITIES:

1.      Provides Home Care nursing services, in accordance with the plan of treatment   signed by the physician, and makes the initial evaluation visit to the patient as needed.

2.      Collaborates with team members, physicians and patients/caregivers to re-evaluate the nursing needs of the patient; initiates, develops, implements and makes necessary revisions to the patient's plan of care.  Assesses the patient's continual care needs.  Addresses all problems in the plan of care or documents rationale for not doing so.  Completes MD orders and 60 day summary according to regulatory guidelines.
