Engage patients in meaningful, life-changing dialogue. You'll be their champion, providing essential education on prevention, risk reduction, sexual health, and substance use. Guide them through treatment adherence, supporting medical appointments and medication regimens, including antiretroviral therapy, with a truly patient-centered, holistic approach.
Be the central figure in managing patient flow, coordinating crucial follow-up appointments, and acting as a vital liaison between patients and medical providers. Facilitate access to essential medical and social support services, ensuring no one falls through the cracks.
Leverage your proficiency with Electronic Medical Records and patient databases to meticulously track patient data, monitor intervention fidelity, and report outcomes. Your analytical skills will ensure our programs are effective, evidence-based, and continuously improving.
Lead daily huddles, proactively identify patients needing follow-up for chronic health maintenance, prevention, or other needs, and coordinate outreach. You'll foster a culture of safety, effective communication, and teamwork that resonates throughout our clinic.
