📝 What You’ll Be Doing

Under the supervision of the Program Director, the Occupational Therapist plays an essential clinical role in JASA’s Living In Place program. Working across in-home, office, and community settings, you will conduct functional ADL/IADL assessments, identify fall prevention needs, review hospital discharge plans, and provide time-limited care management to restore or maintain physical, cognitive, and mental functioning.

You will manage a direct client portfolio, formulating adaptive service plans, tracking clinical progress, and connecting older adults to vital community resources in accordance with JASA and NYC Department of Health and Mental Hygiene (DOHMH) standards.

Key Responsibilities:

Functional Assessments & Service Planning: Conduct comprehensive ADL and IADL assessments; develop, monitor, and update individualized service plans designed to maximize client performance in daily life tasks.

Home Safety & Fall Prevention: Evaluate home environments for safety hazards; recommend fall prevention aids, adaptive equipment, and environmental modifications to ensure clients remain safely at home.

Hospital Discharge & Targeted Interventions: Review hospital discharge plans for recently released clients; deliver time-limited clinical care management and targeted interventions to prevent re-hospitalization.

Care Management & Reassessment: Perform ongoing reassessments of client functional needs, monitor service efficacy, and adjust service plans to address evolving health or cognitive changes.

Community Partnerships & Crisis Oversight: Build cooperative referral relationships with local agencies; identify crisis situations or high-risk cases and escalate to supervisors for immediate guidance.

Compliance & Record-Keeping: Maintain thorough, timely, and audit-ready electronic case notes, progress reports, and statistical documentation in compliance with JASA and DOHMH standards.

Community Representation & Professional Development: Represent JASA at community meetings and conferences; participate in staff meetings and complete required trainings in evidence-based clinical models.

🎓 What You’ll Need

Education & Licensure: Bachelor’s Degree in Occupational Therapy or a post-baccalaureate certificate in Occupational Therapy from an accredited institution, with active New York State (NYS) OT Licensure required.

Clinical Competencies: Knowledge of ADL/IADL evaluation techniques, fall prevention aids, adaptive skills training, and post-acute discharge review for older adult populations.

Field Mobility: Ability to travel across NYC using public transit to conduct home assessments and navigate physical staircases and environmental barriers within client residences.

Technical Skills: Computer literacy, including proficiency with Microsoft Office Suite and the ability to learn and navigate web-based client management software.

Communication & Cultural Competence: Solid writing and record-keeping skills; fluency in English required (Spanish bilingual skills preferred); a deep understanding and appreciation of diversity across race, religion, ethnicity, gender, sexual orientation, and personal values.
