Job Summary:

Carease Health - IL, a nationwide preventative healthcare company, is seeking a reliable and experienced Medical Assistant or Certified Nursing Assistant (CNA) to join our team! As a Medical Assistant, you will play a vital role in our comprehensive preventative care program, which incorporates Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM). Your expertise will enable us to prioritize proactive measures for enhanced physical health and well-being.

Responsibilities:

As a Medical Assistant at Carease Health - IL, you will:

• Take vital signs, such as blood pressure, temperature, and pulse, with precision and accuracy

• Maintain accurate and up-to-date patient records, adhering to HIPAA guidelines

• Collaborate with our experienced clinical team to ensure seamless integration with our facility's processes and protocols

• Utilize our RPM/CCM system to continuously monitor vital signs and detect abnormalities swiftly

• Participate in CoCM elements to proactively approach mental health and promote overall well-being

Requirements:

• Medical Assistant: 2 years of experience (Required)

• Vital signs: 2 years of experience (Required)

• License/Certification:

  • Driver's License (Required)

  • Certified Medical Assistant (Required)

• Ability to Relocate: Relocate before starting work

Schedule:

Monday to Friday
