As a Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine, you will deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities:

• Provide high-quality, patient-centered care to residents in skilled nursing facilities

• Conduct on-site visits and remote patient assessments

• Develop and implement individualized care plans

• Monitor patient progress and adjust care plans as needed

• Collaborate with healthcare teams to ensure seamless patient care

• Maintain accurate and timely documentation

Requirements:

• Active NP or PA license (in the state of practice)

• 2+ years of experience in Geriatric care

• Excellent communication, collaboration, and teamwork skills

• Proven experience in documentation review

About the Role:

• Salary: $17,000 - $19,000 ANNUALLY

• Part-time schedule with flexibility
