Physical Therapist - Outpatient Rehab Clinical Coordinator

Mozaic Senior Life is hiring a Physical Therapist to serve as Clinical Coordinator for its outpatient rehabilitation program. About 75–80% of your time goes to direct patient care and 20–25% to coordination and leadership: daily clinic operations, staff scheduling, program growth and community outreach. The program supports wellness, recovery, functional independence and aging in place. Reports to the VP of Community Services and Business Development.

Responsibilities:
• Maintain an active outpatient caseload and model evidence-based, patient-centered care
• Support therapist scheduling, workflow and clinic operations, including tracking cancellations and no-shows
• Help develop new therapy and wellness programs and grow referrals
• Build relationships with physicians, referral sources and community partners
• Ensure compliance with federal, state, payer and organizational requirements, and support audit readiness
• Guide, mentor and onboard therapy staff

Qualifications:
• Bachelor's degree required; advanced degree preferred
• 3–5 years of outpatient therapy and supervisory experience in geriatric or neurological rehabilitation
• Unrestricted Connecticut Physical Therapist license
• Strong clinical, operational and relationship-building skills
• Able to lift, transfer and reposition patients (up to 50 lbs independently)
