Role Summary:

Carease Health - TN is seeking an experienced Nurse Practitioner (NP) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. As a hybrid role, you'll combine on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities:

• Build strong relationships with skilled nursing facilities and maintain a patient-centered approach

• Collaborate with interdisciplinary teams to deliver high-quality, patient-centered care

• Conduct on-site visits and remote patient monitoring to identify potential health issues and address patient needs

• Document patient encounters efficiently and accurately with the support of a dedicated scribe team

• Participate in training and education sessions to stay up-to-date on best practices and clinical guidelines

Requirements:

• Active NP or PA license in the state of practice

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills
