Help us keep patients connected to the care they need! In this role, you'll use your communication skills, creativity, and problem-solving abilities to address concerns, rebuild relationships, and help patients return to consistent care.

What You'll Be Doing:

• Connect with patients and families to understand their concerns and find solutions
• Use your creativity and problem-solving skills to de-escalate challenging situations
• Build relationships and bring at-risk cases back to active service
