VP of Home Care Operations

Location: Brooklyn, NY
Salary: $175,000–$220,000 DOE

Position Overview

A growing Certified Home Health Agency (CHHA) is seeking a seasoned Director of Operations to lead its overall operations and support continued growth.

This is a hands-on leadership role for someone with a strong background in home healthcare who understands how to manage a complex operation across both office and field teams. The ideal candidate will bring structure, improve coordination between departments, strengthen accountability, and ensure that daily operations run efficiently while meeting all regulatory requirements.

The right person will be comfortable overseeing the full operational cycle, from intake and scheduling through staffing, field services, and billing, while working closely with executive leadership to improve performance and scale the organization.

Key Responsibilities

• Direct daily operations across all office-based and field functions.
• Oversee department leaders and teams responsible for intake, coordination, scheduling, staffing, billing, and field services.
• Establish clear operating procedures, performance standards, and accountability measures.
• Track key performance indicators, identify gaps, and implement solutions to improve productivity and efficiency.
• Ensure effective communication between departments, field personnel, patients, and leadership.
• Build and develop strong teams, providing direction, coaching, and performance management.
• Identify operational bottlenecks and lead initiatives to improve workflows and overall business performance.
• Partner with ownership and executive leadership on strategic planning, organizational growth, and operational priorities.
• Maintain compliance with applicable healthcare regulations, CHHA requirements, and internal policies.
• Develop scalable systems and processes to support the agency’s continued expansion.
• Take ownership of complex operational challenges and drive them through to resolution.
• Promote a professional, collaborative, and results-oriented work environment.

Requirements

• Significant experience in a CHHA or comparable home healthcare organization is required.
• Strong understanding of home healthcare operations, regulatory requirements, and business processes.
• Proven ability to manage multiple departments and large teams.
• Experience overseeing both office personnel and field operations.
• Strong working knowledge of intake, scheduling, staffing, care coordination, billing, and related operational functions.
• Demonstrated ability to lead teams, establish accountability, and improve performance.
• Excellent judgment, problem-solving, and decision-making skills.
• Ability to work independently, prioritize competing demands, and thrive in a fast-paced environment.
• Strong communication, organizational, and leadership skills.
