Carease Health is seeking a dedicated and compassionate Nurse Practitioner to join our team of healthcare professionals in Jackson, TN. As a Nurse Practitioner with our company, you will have the opportunity to deliver high-quality preventative care to residents in skilled nursing facilities while enjoying a hybrid work arrangement that combines on-site visits with remote work from home.

About the Role:

Incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) into our comprehensive preventative care program, we prioritize proactive measures for enhanced physical health and well-being. As a Nurse Practitioner with Carease Health, you will work closely with our experienced clinical team to provide patient-centered care that minimizes hospitalizations and elevates patient care standards.

Responsibilities:

• Deliver high-quality preventative care to residents in skilled nursing facilities

• Conduct on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation

• Build strong relationships with facility staff and maintain a patient-centered approach

• Collaborate with our clinical team to provide comprehensive care to patients

• Stay up-to-date with the latest medical research and guidelines to ensure that patients receive the best possible care

Requirements:

• Active Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of practice

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills
