The Keystone Program is focused on ensuring ABAWD/SNAP clients can access the support and guidance they need to sustain benefits and achieve financial stability.

The Case Manager & Benefits Navigator provides comprehensive case management and individualized support to program participants, with a primary focus on SNAP enrollment, retention, and compliance for ABAWD participants. The position assists participants with navigating SNAP requirements, completing required activities, accessing supportive services, and connecting with volunteer, community service, training, and employment opportunities. The Case Manager & Benefits Navigator works closely with participants to identify barriers, develop service plans, and support progress toward stability and economic self-sufficiency.

 

Job Responsibilities:

  • Provide ongoing case management, participant assessments, individualized service planning, and follow-up.
  • Identify participant needs, strengths, goals, and barriers to program participation, employment, training, and SNAP compliance.
  • Assist ABAWD participants with SNAP enrollment, recertification, and retention of benefits.
  • Educate participants on SNAP and ABAWD requirements and monitor participation to help ensure ongoing compliance.
  • Track required activities and documentation, and follow up with participants regarding outstanding SNAP requirements or potential barriers to compliance.
  • Identify and address barriers related to housing, food insecurity, healthcare, childcare, transportation, and financial stability.
  • Connect participants with supportive services, public benefits, community resources, and emergency assistance when appropriate.
  • Identify volunteer and community service sites and assist participants in securing opportunities that meet program and SNAP requirements.
  • Assist participants in identifying and pursuing appropriate employment, training, and workforce development opportunities.
  • Support participant engagement in employment, volunteer, community service, and training activities.
  • Maintain regular communication with participants to monitor progress and provide guidance and support.
  • Maintain accurate and timely case files, case notes, service plans, referrals, and documentation in accordance with agency, funding, SNAP, and regulatory requirements.
  • Monitor participant outcomes and prepare reports related to engagement, SNAP compliance, employment, volunteer activities, and program services.
  • Ensure compliance with program policies, funding requirements, confidentiality standards, and documentation requirements.
  • Participate in team meetings, case conferences, and trainings.

 

Job Requirements:

  • Bachelor’s degree in Social Work, Human Services, Psychology, Counseling, or a related field; equivalent experience may be considered.
  • 2–3 years of experience in case management, human services, social services, benefits navigation, workforce development, or a related field.
  • Experience with SNAP, SNAP Employment & Training, ABAWD requirements, or similar public assistance programs preferred.
  • Knowledge of public benefits, community resources, supportive services, and employment-related barriers.
  • Experience connecting participants with employment, volunteer, community service, or training opportunities preferred.
  • Strong case management, assessment, problem-solving, advocacy, and communication skills.
  • Ability to establish rapport and engage individuals from diverse backgrounds.
  • Strong organizational skills and attention to detail.
  • Proficiency in Microsoft Office and case management databases.
  • Ability to manage multiple priorities while maintaining accurate and timely documentation.

 

This role is On-Site, and Full-Time, 35 hours per week

Pay: $24.00 - $27.00 per hour
