As a Nurse Practitioner (NP) or Physician Assistant (PA) with Carease Health - PA, you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities. Our unique hybrid model combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. Our goal is to prioritize proactive measures for enhanced physical health and well-being, while also addressing mental health and minimizing hospitalizations.

About the Role:

We are seeking an experienced NP or PA with a background in Geriatrics or Internal Medicine to join our team. As a key member of our clinical team, you will:

• Conduct on-site visits to assess patients' physical and mental health

• Implement remote patient monitoring (RPM) and chronic care management (CCM) programs to prevent hospitalizations

• Collaborate with healthcare providers and facilities to ensure seamless care transitions

• Participate in regular training and professional development opportunities to stay up-to-date on industry best practices

Requirements:

• Active NP or PA license (in the state of practice)

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills
