Case Planner (START)

POSITION SUMMARY

JCCA is seeking a compassionate, organized, and dedicated Case Planner to join our Residential Treatment Campus in Pleasantville, NY. This is an opportunity to play a meaningful role in helping children and adolescents navigate complex challenges, strengthen family connections, and work toward safe and stable permanency.

Our Residential Treatment Campus provides a safe, structured, and supportive environment for youth ages 5–21 who need intensive behavioral, emotional, and developmental support. The campus includes four specialized programs:

Pleasantville Cottage School - serves children with long-term emotional and behavioral challenges

Edenwald Center – serves children who have low IQs, autism and other developmental disorders

START - serves children who need assessment, residential treatment and intensive intervention for a short period of time

Center for Healing - treats children who are the victims of sexual abuse and/or commercial sexual exploitation

As a Case Planner, you will be an essential member of a multidisciplinary team, working closely with youth, families, clinicians, residential staff, schools, community providers, and regulatory agencies. You will help ensure that each young person has an individualized plan focused on safety, well-being, family connections, and permanency.

SUMMARY OF JOB DUTIES:

Case Planning & Family Engagement

• Develop and implement individualized case management, treatment, and discharge plans in collaboration with the youth, family, and multidisciplinary team.
• Participate in Family Team Conferences, Service Plan Reviews, Individual Reviews, and other team meetings.
• Build and maintain collaborative relationships with youth, families, caregivers, and service providers.
• Support and facilitate family and sibling visitation, including developing visitation plans and providing supervision when needed.
• Coordinate required assessments upon admission and support the youth’s transition throughout treatment.

Permanency & Advocacy

• Advocate for the needs and best interests of youth while working toward safe and timely permanency.
• Serve as a liaison with ACS, DSS, CSE, OCFS, and other regulatory agencies, as well as attorneys and officers of the court.
• Represent the team at Family Court and QRTP hearings and communicate court outcomes and follow-up needs to the treatment team.
• Collaborate with community providers to support successful and timely discharge from the QRTP setting.
• Identify, coordinate, and follow up on community resources and services needed by youth and families.

Youth Support & Crisis Response

• Work closely with Clinical and Child Care teams to support youth during periods of crisis.
• Participate in crisis intervention using a trauma-informed, strength-based, and youth-centered approach.
• Support implementation of Absent Without Consent (AWOC) and Commercial Sexual Exploitation of Children (CSEC) protocols in accordance with JCCA and regulatory requirements.
• Promote youth voice, family engagement, and positive outcomes throughout the treatment process.

Documentation & Compliance

• Prepare and maintain required case documentation, including Family Assessment Service Plans (FASP), court reports, Permanency Hearing Reports (PHR), Termination of Parental Rights (TPR) summaries, surrender applications, transfer summaries, incident reports, and briefings.
• Maintain accurate case records and documentation in accordance with agency policies and regulatory requirements.
• Complete progress notes in Connections and/or electronic health record systems.
• Maintain accurate placement information and document placement movements and other case activity.
• Ensure required documentation is completed accurately and within established deadlines.

 

QUALIFICATIONS:

• Bachelor's Degree in social work, human services, psychology or related field; Master's degree preferred
• Experience working with families and adolescents in the child welfare system is preferred
• Strong interpersonal skills and the ability to establish trusting, professional relationships with youth and families.
• Ability to work with diverse population of clients and team members
• Excellent organizational skills and the ability to manage multiple priorities and meet deadlines.
• Must have excellent verbal and written communication skills.
• Must have a commitment to work from a strength-based, youth development and trauma informed perspective.
• Computer proficiency and the ability to learn and effectively use new systems.
• Bilingual English/Spanish preferred.
• NYS Drivers Licensed preferred to access Agency vehicles; NJ and CT license is acceptable

 

*Ability to work flexible hours - some evenings and occasional weekends as needed to meet client needs

COMPENSATION:

Bachelor Degree - $27.47 -$29.71 per hour  ($50K -$54K annualized)

Master's Degree - $32.05 - $33.34 per hour ($58K - $60.8K annualized)

Differentials offered for Spanish language skills and/or NYS LMSW, LMHC, LMFT or LCAT.
