Patient Case Manager

HIRING!

Pay: $50,000–$55,000

Location: Crown Heights — 4-6 weeks full-time, in-person training, then remote but based in New York

Schedule: Mon–Fri, 9–5 availability

As a Case Manager, you'll coordinate the moving pieces so that everything runs smoothly for your caseload of 25 patients and their family members. You'll be the point person helping them navigate challenges as they arise, making sure nothing slips through the cracks.

What you'll be doing:

• Check in with patients over the phone regularly to monitor their status
• De-escalate and problem-solve when patient issues arise
• Complete state paperwork accurately and ensure all deadlines are met
• Conduct 8 home visits monthly across the 5 boroughs — must be comfortable going into patients' homes
• Build trusted relationships with patients and families over time

BA or Associate's degree required!
