Service Coordinator

Position Overview

We are seeking a compassionate and detail-oriented Patient Liaison to oversee a caseload of approximately 25 patients participating in the NHTD program. The individuals served are living with dementia or cognitive decline and require consistent coordination, advocacy, and support. This role offers meaningful, hands-on experience within a collaborative and supportive healthcare environment.

Responsibilities

• Conduct regular phone check-ins with families and caregivers
• De-escalate and resolve concerns with service providers
• Schedule therapies and transportation
• Complete renewal paperwork and incident reports
• Conduct in-home visits once per quarter throughout the five boroughs

Training & Schedule

• 4–6 weeks of paid, in-person training in Brooklyn
• Hybrid schedule after training (remote work + quarterly home visits throughout the 5 Boroughs)

Qualifications

• Associate’s or Bachelor’s degree required
• Strong communication and organizational skills
• Related healthcare or human services experience is a plus, but not required
