JOB SUMMARY
We are seeking a Keystone Case Manager to join its Career Services Department. The Case Manager will provide individualized support to program participants as they work toward employment, education, training, and long-term career goals.

We are seeking a Keystone Case Manager to join its Career Services Department. The Case Manager will provide individualized support to program participants as they work toward employment, education, training, and long-term career goals.

WHAT YOU’LL DO
- Conduct participant intake, eligibility screening, and comprehensive needs assessments.
- Develop individualized service plans based on each participant’s employment, education, and career goals.
- Provide ongoing case management, career coaching, and motivational support.
- Assist participants with résumé development, job applications, interview preparation, workplace readiness, and job-search strategies.
- Connect participants to employment opportunities, occupational training, educational programs, benefits, and supportive services.
- Identify barriers to employment and coordinate appropriate referrals to internal and community-based resources.
- Maintain consistent communication with participants and conduct timely follow-up.
- Monitor participant progress toward established goals and required program milestones.
- Maintain accurate, confidential, and timely case notes, participant files, attendance records, referrals, and outcome documentation.
- Enter and update participant information in required databases and tracking systems.
- Support participant recruitment, outreach, orientation, workshops, and program events.
- Collaborate with Career Services staff, employers, training providers, educational institutions, and community partners.
- Assist with employment placement, retention tracking, and follow-up services.
- Ensure that services and documentation comply with grant requirements, agency policies, and established deadlines.
- Participate in team meetings, case conferences, professional development, and program evaluation activities.
- Perform other related duties as assigned.
OUR REQUIREMENTS
- Bachelor’s degree in social work, human services, psychology, counseling, education, workforce development, or a related field.
- At least two years of relevant experience in case management, workforce development, career coaching, employment services, or human services.
- Demonstrated ability to engage and build trust with individuals from diverse backgrounds.
- Strong organizational, time-management, and follow-up skills.
- Excellent verbal, written, and interpersonal communication skills.
- Ability to manage multiple participants, priorities, and deadlines.
- Experience maintaining confidential records and completing detailed case documentation.
- Proficiency with Microsoft Office, Google Workspace, databases, and electronic case-management systems.
- Ability to work independently while contributing to a collaborative team environment.
PREFERRED REQUIREMENTS
- Experience working in a grant-funded workforce or career-services program.
- Knowledge of Staten Island employment, education, training, and supportive-service resources.
- Experience supporting individuals who face barriers to employment.
- Familiarity with trauma-informed, strengths-based, and person-centered practices.
- Experience facilitating job-readiness workshops or group sessions.
- Bilingual or multilingual skills are a plus.
