Case Manager

Responsibilities Include:

Serve as a steady, trusted guide for families navigating complex pediatric diagnoses – building deep relationships that ensure no parent or child feels alone in moments of uncertainty or crisis

Coordinate and deliver personalized support plans that integrate medical, emotional, and practical needs – bridging gaps between hospitals, schools, and community resources

Build and maintain a robust network of providers, specialists, and community partners – creating a high-quality referral ecosystem that connects families to the right support at the right time

Advocate on behalf of families in healthcare and educational settings, helping them access critical services, ask the right questions, and make informed decisions

Respond to urgent, real-time needs with compassion and precision – mobilizing resources quickly and effectively when timing matters most, while empowering families toward long-term stability and independence

Required Experience:

A healthcare-related master’s degree (such as social work, human services, public health, or healthcare navigation) is preferred but not required. We are looking for someone who is compassionate, resourceful, hardworking, and proactive.
