Home Care Agency seeking Director of Coordination to oversee care coordination operations, caregiver staffing, patient coverage, compliance, and team performance. 5+ years of Home Care leadership/management experience, strong knowledge of NYS home care regulations and labor requirements, experience managing high-volume coordination teams, and HHA Exchange experience required.
