Position Overview
A growing healthcare advocacy organization is seeking a compassionate, high-energy Patient Care Manager & Hospital Liaison to serve as a trusted resource for patients and families facing medical challenges. Reporting directly to the CEO and working closely with the COO and Care Coordination team, this individual will help patients navigate complex healthcare situations, coordinate access to appropriate resources, and provide support during urgent and time-sensitive circumstances.
The ideal candidate is a heimish, people-oriented professional who is passionate about helping others, builds relationships naturally, and takes ownership of ensuring every patient receives the guidance and advocacy they need.
Responsibilities
- Serve as a primary point of contact for patients and families seeking assistance
- Assess each situation and ensure patients are connected to the appropriate internal department, specialist, or resource
- Advocate for patients when challenges or barriers arise with hospitals, providers, insurance companies, or healthcare systems
- Coordinate communication between patients, families, physicians, hospitals, and internal team members
- Help facilitate hospital transfers, specialist referrals, and other time-sensitive care needs
- Build and maintain strong relationships with physicians, hospital staff, care managers, and healthcare organizations
- Work closely with the CEO, COO, and coordinators to ensure seamless case management and follow-through
- Monitor active cases and ensure patients receive timely updates and ongoing support
- Assist with urgent and after-hours situations when needed
- Maintain accurate records and case documentation
Qualifications
- 3+ years of operations, care coordination, healthcare administration, patient advocacy, or related experience
- Healthcare, medical, EMS, nursing, or similar background preferred
- Fluent in English and Yiddish
- Exceptional interpersonal and communication skills
- Strong organizational abilities with excellent attention to detail
- Ability to manage multiple priorities in a fast-paced environment
- Resourceful problem-solver who remains calm under pressure
- Passion for helping people and making a meaningful impact
- Self-starter with a strong sense of ownership and accountability
- Comfortable being available to assist with urgent situations outside traditional business hours
Compensation & Benefits
