Responsibilities
- Coordinate services and community resources for clients
- Communicate with clients, families, and service providers
- Manage and monitor an assigned caseload
- Conduct quarterly home visits to assess client needs
- Maintain accurate documentation and required reports
- Follow up to ensure services are being delivered properly
Qualifications
- Associate or bachelor’s degree required
- Strong communication and people skills
- Organized, responsible, and detail-oriented
- Able to manage multiple clients and priorities
- Strong follow-through and documentation skills
- Comfortable working independently and as part of a team
