Location: Monsey, NY
Schedule: Monday–Thursday 9:00 AM–5:00 PM | Friday 9:00 AM–1:00 PM
Type: Full-Time | On-Site
Travel: Occasional travel to regional offices

Position Overview

A growing New York home care agency is seeking an experienced Director of Patient Services (DPS) to lead its clinical operations.

This is a senior leadership role for a New York State Registered Nurse with strong home care experience and a proven ability to manage clinical teams, oversee multiple service lines, maintain regulatory compliance, and drive operational improvements.

The ideal candidate is a calm, hands-on, business-minded clinical leader who can balance patient care, compliance, staffing, efficiency, and agency growth.

Key Responsibilities

  • Lead and oversee all clinical and nursing operations.
  • Manage and develop RN Supervisors, RNs, LPNs, field nurses, nursing coordinators, aides, and clinical support staff.
  • Oversee clinical operations across LHCSA/PCA, PDN, and pediatric/CHHA-related programs.
  • Ensure plans of care, physician orders, 485s, authorizations, documentation, and supervisory requirements are properly maintained.
  • Provide leadership on complex, pediatric, PDN, and high-acuity cases.
  • Oversee clinical documentation, quality assurance, incidents, complaints, admissions, reassessments, and discharges.
  • Take ownership of DOH audits, surveys, investigations, and corrective action plans.
  • Maintain agency readiness for regulatory reviews and ensure deficiencies are addressed promptly.
  • Develop, update, and implement clinical policies, procedures, and workflows.
  • Oversee PCA/HHA training and certification programs.
  • Monitor clinical performance and implement improvements to strengthen quality, compliance, and efficiency.
  • Work closely with management and other departments to improve operations and support agency growth.

Qualifications

  • Active New York State RN license required.
  • Strong home care experience with DPS or senior clinical leadership experience preferred.
  • Proven experience managing and supervising clinical teams.
  • Strong knowledge of LHCSA, CHHA, PCA, PDN, DOH regulations, and home care compliance.
  • Experience with DOH audits, surveys, investigations, and corrective action plans.
  • Strong understanding of clinical documentation, plans of care, 485s, and quality assurance.
  • Excellent leadership, communication, organizational, and problem-solving skills.
  • Ability to manage multiple programs and priorities in a fast-paced environment.
  • Practical, accountable, professional, and solutions-oriented leadership style.
