Patient Outreach Coordinator - Salary: $50,000–$60,000

Schedule: Full Time, Mon-Fri, 9am–5pm

Location: Williamsburg, Brooklyn (in-person) - Shuttle from Crown Heights provided

What you'll be doing:

•  Figuring out why a patient's care paused or is at risk
•  Having conversations with patients & families to build trust and relationships
•  Teaming up with staffing & clinical teams to solve problems fast
•  Owning your cases from first call to full resolution
• *Following up consistently until each case is fully resolved
