Today, it is the largest provider of licensed outpatient community mental health services and one of the largest human service agencies in Westchester County. WJCS delivers innovative programs and services and compassionate care to more than 20,000 people of all ages and backgrounds throughout the county.</span></p><p style='margin:0in;font-size:13px;font-family:"Times New Roman",serif;'><br></p><p style='margin:0in;font-size:13px;font-family:"Times New Roman",serif;'><span style="font-size:16px;">We are currently seeking a compassionate and organized full-time (35 hours per week) Care Manager to join our Home Delivered Meals Program. In this role, you will offer services, including in-home assessments, case management, care planning, information and referrals to the seniors participating in the program. team, your responsibilities will include, but are not limited to:</span></p><ul style="margin-bottom:0in;margin-top:0in;" type="disc"><li style='margin:0in;font-size:13px;font-family:"Times New Roman",serif;'><span style="font-size:16px;">Conduct comprehensive in-home assessment, including financial assessments, to determine eligibility for the grant, programs, services and entitlements.</span></li><li style='margin:0in;font-size:13px;font-family:"Times New Roman",serif;color:black;'><span style="font-size:16px;color:windowtext;">Complete home visits throughout the Westchester County community.</span></li><li style='margin:0in;font-size:13px;font-family:"Times New Roman",serif;'><span style="font-size:16px;">Conduct annual reassessments, as well as reassessments whenever necessary.</span></li><li style='margin:0in;font-size:13px;font-family:"Times New Roman",serif;'><span style="font-size:16px;">Develop individualized client Care Plans. </span></li><li style='margin:0in;font-size:13px;font-family:"Times New Roman",serif;'><span style="font-size:16px;">Ensure clients and their caregivers receive information about and assistance with appropriate referrals.</span></li><li style='margin:0in;font-size:13px;font-family:"Times New Roman",serif;'><span style="font-size:16px;">Maintain accurate, timely and complete documentation in accordance with Roman",serif;margin-left:9.0pt;text-indent:-9.0pt;'><span style="font-size:16px;">We are seeking candidates who possess the following qualifications:</span></p><ul style="margin-bottom:0in;margin-top:0in;" type="disc"><li style='margin:0in;font-size:13px;font-family:"Times New Roman",serif;'><span style="font-size:16px;">Bachelor’s Degree in a human services field</span></li><li style='margin:0in;font-size:13px;font-family:"Times New Roman",serif;'><span style="font-size:16px;">Experience working with senior citizens</span></li><li style='margin:0in;font-size:13px;font-family:"Times New Roman",serif;'><span style="font-size:16px;">Bilingual English/Spanish is a plus</span></li><li style='margin:0in;font-size:13px;font-family:"Times New Roman",serif;'><span style="font-size:16px;">Valid driver’s license and vehicle </span></li><li
