Integrated Health Care Manager

This position is for case management for clients stuck in a dysfunctional life cycle due to mental health challenges and adverse life circumstances; responsibilities include linking clients to community and government eligibility programs, strategizing with family, collaborating with therapists, psychiatrists, and hospitals when applicable, and being there with a kind heart, warm smile, and a message of hope. Requirements: experience in mental health and care management and a BA degree are required for this position, along with the ability to connect with people from various backgrounds and access to a car.
