Description
The Employment Services Coordinator / Case Manager is responsible for supporting individuals participating in employment and workforce development programs by coordinating community service placements, providing comprehensive case management, and assisting participants in overcoming barriers to employment. This role develops and maintains partnerships with community organizations, connects participants to volunteer and employment opportunities, monitors program compliance, and provides individualized support to help clients achieve greater economic self-sufficiency.

Job Responsibilities:

Provide comprehensive case management services to program participants through ongoing assessment, goal setting, and individualized service planning.
Identify and address barriers that may interfere with successful program participation, employment, or training.
Connect participants with supportive services, including housing, food assistance, healthcare, childcare, transportation, and other community resources.
Assist participants in maintaining eligibility for SNAP and other public benefits by ensuring compliance with program requirements.
Support clients in accessing emergency financial assistance and other available resources when appropriate.
Maintain regular contact with participants to monitor progress and provide ongoing guidance and encouragement.
Develop, establish, and maintain relationships with community organizations and volunteer sites to create meaningful placement opportunities.
Coordinate community service assignments that align with program requirements and participant skills, interests, and career goals.
Monitor participant attendance, performance, and compliance at volunteer and community service sites.
Maintain accurate documentation of placements, participation, outcomes, and employer/community partner communications.
Identify and refer participants to employment opportunities, job training, educational programs, and workforce development services.
Collaborate with employers and community partners to identify job openings and promote participant employment.
Maintain complete, accurate, and timely case files in accordance with agency, funding source, and regulatory requirements.
Document participant activities, services provided, case notes, referrals, and outcomes.
Prepare reports and track performance metrics related to placements, participation, and employment outcomes.
Ensure compliance with all program policies, funding requirements, and confidentiality standards.
Participate in team meetings, case conferences, and professional development opportunities.

Job Requirements:

Bachelor's degree in Social Work, Human Services, Psychology, Counseling, or a related field; equivalent experience may be considered.
2-3 years of experience in case management, workforce development, employment services, or human services.
Experience working with public assistance programs, including SNAP Employment & Training or similar workforce initiatives, preferred.
Knowledge of community resources, employment services, and benefit programs.
Strong interpersonal, organizational, and problem-solving skills.
Ability to build partnerships with community organizations and employers.
Excellent written and verbal communication skills.
Proficiency in Microsoft Office and case management databases.
Ability to manage multiple priorities while maintaining accurate documentation.
