Job Description:

We are seeking a seasoned clinical leader to serve as our VP of Clinical Nursing. As the senior clinical authority and clinical architect, you will play a pivotal role in designing, governing, and continuously improving the clinical framework across all of our CPT-based programs and service lines.

About the Role:

As the VP of Clinical Nursing, you will be responsible for translating each facility's clinical priorities into scalable protocols, treatment pathways, and documentation standards. You will work closely with our Clinical Operations team to execute day-to-day delivery, ensuring that our clinical strategies and policies align with the needs of our facility partners.

Responsibilities:

• Facility-Centric Goals & Protocols: Identify and translate facility clinical priorities into monthly and quarterly goals, create and approve clinical protocols, and define daily and weekly workflows.

• Facility & Provider Clinical Governance: Define our clinical engagement model, establish standards for collaboration with facility physicians, and serve as the final clinical authority on standards of care.

• Leadership & Oversight: Provide direct clinical oversight to Director(s) of Clinical Operations, maintain a clear boundary between clinical governance and operational execution, and serve as the final escalation point for clinical governance decisions.

What Success Looks Like:

• Facilities are satisfied with reports and find them clear and actionable.

• Each facility has monthly and quarterly goals with clear KPIs, with progress tracked over time.

• Clinical Operations execution teams work from clear playbooks with defined success metrics.

• Facilities receive risk reports and information that help them act on emerging issues.

Requirements:

• Significant clinical leadership experience, ideally spanning skilled nursing, post-acute, or facility-based care settings.

• Deep familiarity with CMS and payer requirements for RPM, CCM, APCM, and/or CoCM programs.

• Proven ability to build and scale clinical protocols across multiple sites or facilities.

• Strong track record partnering with facility leadership and physician teams.

• Active clinical licensure in good standing.

• Executive presence and the ability to serve as a final clinical decision-maker at scale.

Compensation & Benefits:

• Base salary: $225,000 - $250,000/year

Work Location: Hybrid-based in or near Lakewood, NJ, with travel to partner facilities as needed.
