About the Role:

As a Nurse Practitioner with Carease Health, you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities. Our team is dedicated to improving the lives of nursing home patients through remote monitoring, chronic care management, and psychiatric collaborative care. Responsibilities:

• Provide high-quality preventative care to residents in skilled nursing facilities

• Conduct on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation

• Build strong relationships with facilities and maintain a patient-centered approach

• Collaborate with the clinical team to ensure seamless care coordination

Requirements:

• Active NP or PA license in the state of practice

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills
