About the Role

We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities:

  • Provide evidence-based, patient-centered care to residents in skilled nursing facilities

  • Conduct on-site visits and remote patient monitoring to detect abnormalities and prevent hospitalizations

  • Collaborate with care teams to develop and implement individualized care plans

  • Maintain accurate and timely documentation of patient care and services

  • Participate in quality improvement initiatives to enhance patient outcomes and facility ratings

Requirements:

  • Active NP or PA license (in the state of practice)

  • 1+ year of experience in Geriatric care and Documentation review

  • Excellent communication, collaboration, and teamwork skills

  • Ability to work independently and as part of a remote team
