Job Summary:

Carease Health is seeking a dedicated and compassionate Nurse Practitioner (NP) to join our team of healthcare professionals. As a hybrid role, you will provide high-quality preventative care to residents in skilled nursing facilities, combining on-site visits with remote work from home. Our experienced clinical team will provide support and training to ensure your success in this role.

Responsibilities:

• Provide preventative care to residents in skilled nursing facilities, focusing on proactive measures for enhanced physical and mental health and well-being

• Conduct on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation

• Build strong relationships with facility staff and maintain a patient-centered approach

• Collaborate with our clinical directors, regional managers, and supervisors to ensure seamless integration with facility processes and protocols

• Participate in documentation review and continuous quality improvement initiatives

Requirements:

• Active NP or PA license in the state of Texas

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills

• Ability to work independently and as part of a team
