Job Summary:

Carease Health - MA is seeking a dedicated and compassionate Social Worker/Care Companion to join our team! As a Social Worker/Care Companion, you will work closely with our experienced clinical team to provide comprehensive preventative care to nursing home patients. This is a high-volume setting, requiring strong organizational skills and the ability to work efficiently in a fast-paced environment. Our ideal candidate will have at least 1 year of experience as a Medical Assistant or Certified Nursing Assistant (CNA) and be skilled in taking vital signs.

Responsibilities:

• Conduct interviews and assessments with patients to identify social, emotional, and psychological needs
• Develop and implement individualized care plans to promote patient well-being and independence
• Provide emotional support and counseling to patients and their families
• Collaborate with healthcare providers to ensure coordinated care and address patient needs
• Monitor patient progress and adjust care plans as needed
• Participate in interdisciplinary team meetings to share information and develop strategies for patient care
• Keep accurate and detailed records of patient interactions and care plans

Requirements:

• Associate's degree in Social Work or related field
• At least 1 year of experience as a Medical Assistant or Certified Nursing Assistant (CNA)
• Skilled in taking vital signs
• Strong communication and interpersonal skills
• Ability to work efficiently in a fast-paced environment
• Reliable and able to maintain a stable schedule, Monday through Friday, with no weekend work
• Driver's License (Required)
• Certified Medical Assistant (Required)

Benefits:

• Competitive salary range: $57,000 - $59,000 ANNUALLY
