We are seeking a compassionate and dedicated Mental Health Provider to join our Collaborative Care team. As a Social Work/Care Companion, you will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.
About the Role:
In this part-time position, you will visit designated facilities in accordance with the schedule set with your supervisor. You will conduct assessments of residents to identify emotional, cognitive, and psychosocial needs, and implement short-term, solution-focused therapy and supportive interventions to address immediate concerns. You will also document visit notes and maintain detailed records of interactions and interventions after each visit.
Key Responsibilities:
Visit designated facilities in accordance with the schedule set with your supervisor
Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
Document visit notes and maintain detailed records of interactions and interventions after each visit
Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP
Requirements:
Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field
Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
Strong communication, documentation, and relationship-building skills
Education: Associate Degree
Experience: 1 year
