Are you a compassionate and dedicated Mental Health Provider looking to make a direct impact on the lives of older adults? Carease Health is seeking a caring and empathetic individual to join our Collaborative Care team as a Social Worker/Care Companion. In this role, you'll provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.

About the Role

As a Social Worker/Care Companion, you'll be responsible for:

• Visiting designated facilities in accordance with the schedule set with your supervisor

• Conducting assessments of residents to identify emotional, cognitive, and psychosocial needs

• Implementing short-term, solution-focused therapy and supportive interventions to address immediate concerns

• Documenting visit notes and maintaining detailed records of interactions and interventions after each visit

• Collaborating with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Requirements

• Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field

• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings

• Strong communication, documentation, and relationship-building skills

• Education: Associate Degree

• Experience: 1 year
