Job Summary:

As a compassionate and dedicated Social Worker/Care Companion, you will be an integral part of our Collaborative Care team at Carease Health - MI. In this rewarding role, you will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. We are seeking a motivated and experienced professional to join our team, working approximately 16 hours per week, Monday-Friday, within regular 8-5 hours.

Key Responsibilities:

• Visit designated facilities in accordance with the schedule set with your supervisor
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
• Document visit notes and maintain detailed records of interactions and interventions after each visit
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Qualifications:

• Associate Degree in Social Work, Psychology, Counseling, or another mental health–related field
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
• Strong communication, documentation, and relationship-building skills
