Join our team at Rockaway Home Care, a nonprofit 501(c)(3) home care agency dedicated to providing high-quality, compassionate care to patients in their homes and communities. We are seeking an organized, professional, and compassionate PCA Coordinator to support our home care team.

About the Opportunity

Rockaway Home Care is looking for a detail-oriented and reliable PCA Coordinator to join our team. This role is ideal for someone who enjoys working with caregivers, supporting clients, and ensuring smooth day-to-day coordination of care. As a PCA Coordinator, you will play a key role in supporting both caregivers and clients by helping match the right aide to the right case, maintaining communication, and ensuring care is properly coordinated.

Key Responsibilities:

• Coordinate PCA/HHA schedules and client assignments
• Communicate with caregivers, clients, families, and office staff
• Help fill open shifts and address schedule changes
• Support caregivers with questions, updates, and assignment details
• Ensure clients receive reliable and consistent care
• Track caregiver availability, attendance, and call-outs
• Assist with onboarding and placement of new caregivers
• Maintain accurate records and update scheduling systems as needed
• Work closely with nursing, intake, HR, and administrative teams
• Help resolve day-to-day scheduling and care coordination issues professionally
• Provide a supportive and respectful experience for caregivers and clients

Requirements:

• Prior home care coordination, scheduling, or administrative experience preferred
• Strong communication and customer service skills
• Ability to multitask and work in a fast-paced environment
• Comfortable speaking with caregivers, clients, and families
• Organized, detail-oriented, and reliable
• Basic computer skills and ability to learn scheduling systems
• Professional, patient, and solution-focused
• Must meet all company hiring and onboarding requirements

Preferred Qualifications:

• Experience working with PCA/HHA caregivers
• Knowledge of home care, Medicaid, MLTC, or community-based care
• Creole and Spanish-speaking candidates strongly preferred
• Bilingual or multilingual skills are a plus
• Prior experience in staffing, scheduling, HR, or healthcare administration
• Strong problem-solving and follow-up skills
