Rockaway Home Care, a division of Foundation For the Elderly, is seeking a dedicated and compassionate Home Care Intake Specialist to join our Inwood office. As a Home Care Intake Specialist, you will play a critical role in processing patient referrals and ensuring a smooth transition into home care services. Role Overview:
As a Home Care Intake Specialist, you will be responsible for receiving and processing referrals from referral sources, contacting patients and families to explain services and gather required information, verifying patient insurance and determining eligibility, and educating patients and families on home care services. You will also work closely with referral sources, MLTC plans, Medicaid assistance resources, and internal teams to ensure each case moves forward properly.

Key Responsibilities:

Receive and process referrals from referral sources

Contact patients and families to explain services and gather required information

Verify patient insurance and determine eligibility

Educate patients and families on home care services

Send referrals to MLTC plans for potential enrollment or Medicaid assistance

Follow up on insurance approvals and determine which services were approved

Ensure Rockaway Home Care receives proper authorization for MLTC cases

Provide case details and updates to the Coordination Department

Communicate with referral sources, patients, families, and community partners

Maintain accurate records and follow up on pending cases

Help ensure a smooth start-of-care experience for each patient

Requirements:

Minimum 1 year of home care intake experience

Understanding of MLTC requirements and home care intake processes

Experience with Medicare, Medicaid, or MLTC insurance preferred

Strong communication skills with patients, families, employees, and referral partners

Computer-efficient and comfortable learning new systems

Highly organized, punctual, and detail-oriented

Strong written and verbal communication skills

Strong analytical, investigative, and critical thinking skills

Self-motivated and able to work with minimal supervision

Able to take initiative and follow up consistently

Professional, reliable, and team-oriented
