Job description

We are seeking an experienced and motivated Clinical Nutrition Manager / Registered Dietitian (RD) to lead and support our clinical nutrition program in a Skilled Nursing and Rehabilitation setting.

The Clinical Nutrition Manager will play a key leadership role in ensuring exceptional nutritional care, regulatory compliance, and positive resident outcomes. This position will work closely with nursing, dietary services, physicians, and the interdisciplinary team.

Responsibilities
Lead and oversee the facility's clinical nutrition program.

Conduct comprehensive nutritional assessments and reassessments.

Develop, implement, and monitor individualized nutrition care plans.

Monitor residents' nutritional status, weight changes, dietary intake, hydration, and other clinical indicators.

Provide clinical oversight and support for residents with complex nutritional needs.

Collaborate with physicians, nursing, speech therapy, dietary services, and the interdisciplinary team.

Participate in care plan meetings and resident care conferences.

Monitor and evaluate therapeutic diets, supplements, tube feeding, and specialized nutritional interventions.

Support wound healing, weight management, hydration, and other clinical nutrition initiatives.

Ensure accurate and timely clinical documentation.

Maintain compliance with federal, state, and facility regulations.

Participate in QAPI, quality improvement, and survey readiness initiatives.

Identify clinical trends and opportunities for improvement and implement appropriate action plans.

Provide education and guidance to staff, residents, and families on nutrition-related matters.

Work collaboratively with the Food Service Director and dietary team to ensure quality and compliance.

Qualifications
Registered Dietitian (RD) credential required.

Active state licensure or certification, where applicable.

Previous experience in a Skilled Nursing Facility, Long-Term Care, Rehabilitation, or healthcare setting strongly preferred.

Previous leadership or management experience preferred.

Strong knowledge of clinical nutrition, therapeutic diets, enteral nutrition, and regulatory requirements.

Experience with MDS, care planning, clinical documentation, and interdisciplinary collaboration preferred.

Excellent leadership, organizational, communication, and critical-thinking skills.

Ability to manage multiple priorities and work effectively in a fast-paced healthcare environment.
